Certification requirements Preferred:
Ambulatory Care Pharmacy Specialty Certification (BCACP)
Minimum years of experience Preferred:
3 years of inpatient or ambulatory pharmacy experience, or completion of PGY 1
Requirements
Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).
Experience: Five years of professionally responsible pharmacist experience required for Bachelor's degree holders. One year of recent ambulatory experience as a pharmacist or pharmacy intern for PharmD candidates.
Certification: BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire).
License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.
Other: Strong communication and interpersonal skills, data entry skills, analytical and math skills required.

- The LPG Pharmacist will be integrated into primary care with a focus on improving patient outcomes through assuring optimal prescribing practices consistent with evidence-based literature and treatment guidelines.
- Conduct medication use evaluations, and support patient access and adherence to therapy.
- Utilize and leverage analytic data to determine which patients will benefit from interventions, medication education and collaborate with primary care providers and specialists in the care of these patients.
- Perform detailed medication reconciliation.
- Conduct virtual or face to face visits to provide comprehensive medication management.
- Provide recommendations to providers regarding utilization of high-cost medications.
- Participate in the development of quality measures and quality improvement initiatives.
- Provide educational presentations to providers.
- Provide technician supervision to ensure accurate and timely submissions of prior authorizations.
